The service
MedNote is a service of YMH LLC for keeping, organizing and sharing your personal health information with the people you choose.
Not medical care and not an emergency service
MedNote does not provide diagnosis, treatment or medical advice and does not replace seeing a doctor. Any opinion from a doctor who views your record is that doctor's responsibility. In an emergency, call your local emergency number. Do not rely on MedNote.
Your account
You sign in with an account from a third party such as Google, Microsoft, Facebook or Apple, and you are responsible for keeping that account secure. You must be of the age of majority in your country to create an account.
Your content
What you upload or write remains yours. You give us only the permission needed to store it and show it to you and to the people you share it with. You are responsible for the accuracy of what you upload and for having the right to upload it. Don't upload another person's health information without their permission or legal authority.
Sharing
You decide who you share with. Check who the person is before sharing, and remember they may keep a copy of what they saw.
Acceptable use
- Don't use MedNote for anything illegal.
- Don't try to access other people's accounts or data.
- Don't upload malware or abusive content unrelated to health.
Availability
We aim to keep the service available, but interruptions may happen for maintenance or reasons outside our control. Keep copies of your original documents.
Ending your account
You can delete your account at any time. We may suspend accounts that break these terms.
Limitation of liability
The service is provided as is. To the extent the law allows, we are not liable for indirect damages arising from its use.
